Assessment guide
Ratings use an illustrative 5 × 5 matrix: likelihood × consequence. Low 1–4, Moderate 5–9, High 10–16, Extreme 17–25. Agree suitable definitions and response priorities for your project or workplace.
Initial ratings describe the risk with existing controls. Residual ratings are your assessment after planned controls; they do not confirm those controls are effective. Use the status, review date and review notes to record follow-up.
OHS control planning
Consider eliminating the hazard first, then substitution, isolation and engineering controls, followed by administrative controls and PPE. Consult affected workers and review controls after changes or incidents.
This register is a planning aid. It does not certify a workplace as safe or replace a task-specific assessment. Safe Work Australia: managing risks.
